Meet your provider //

Hi, I'm Angelique Samuel, MSN, FNP-C

My journey in nursing began in 2007 as an LPN, and every step since has deepened my commitment to patient care. I went on to earn my RN, then my Bachelor of Science in Nursing, and finally my Master of Science in Nursing from Chamberlain College of Nursing, becoming a Family Nurse Practitioner in 2017. Each stage of that path gave me a different vantage point on what patients and families truly need.

Before becoming a nurse practitioner, I spent years at the bedside caring for critically ill patients in the Medical ICU at UF Health Jacksonville, and served as Director of Nursing for an 84-bed skilled nursing facility — where I also led risk management, infection control, and staff education. I've trained nursing leaders across facilities, piloted programs to reduce hospital readmissions, and earned certification as an Alzheimer's and dementia trainer. That leadership experience taught me something the textbooks can't: great care is about systems, communication, and genuinely listening to people.

As a nurse practitioner, I've cared for patients in nearly every setting — hospitals, skilled nursing facilities, assisted living communities, primary care offices, and virtually through my own telehealth practice. I currently manage complex chronic conditions with a proactive approach: identifying changes early, educating patients and families, and preventing the hospitalizations that so often disrupt lives.

Outside of medicine, my faith and my family are the foundation of everything I do. They keep me grounded, remind me why I chose this calling, and shape the way I care for others — with compassion, patience, and purpose. I believe caring for people is more than a career; it's what I was made to do.

What drives me is simple. I believe healthcare should be personal, accessible, and built around you — not the other way around. Whether I'm seeing you in your home, at your community, or over video, you'll get a provider who takes the time to know you, explains things clearly, and treats you like a person, not a chart.

I'm licensed in nine states — Maryland, Pennsylvania, Virginia, Delaware, Georgia, Florida, South Carolina, North Carolina, and Alabama — and I practice autonomously, so I can meet you wherever you are.

Ready to experience healthcare that's built around you? I'd love to meet you — book a visit today and let's get started.
